We offer coordination services for people living with HIV who seek to obtain and/or maintain an undetectable viral load. Our members (or Stars as we call them) set and work towards healthy living goals with trained program staff. Case managed Stars receive assistance with medication adherence, accessing medical care, treatment, insurance coverage, dental care and linkage to other needed resources such as mental health, housing, substance use treatment and more.
